Job Responsibilities:

• You will receive a monthly work allocation; you’ll need to contact individuals and their families to arrange assessment appointments.
• You’ll assess people in their own homes, prepare written reports of the assessment, identify needs, plan, agree outcomes and take action to implement interventions.
• As a ‘Housing Solutions’ resource much of our work is around adaptation to property or support and assisting local housing authorities to identify and assess suitability of alternative accommodation more suited to long-term needs.
• Working as part of a wider team, you’ll partnership with our colleagues in HCC, NHS and local housing authorities across Hertfordshire, to provide assessment and recommendations for adult care.
• This will include advising on major/minor adaptations, Disabled Facilities Grants, moving and handling and specialist equipment required.

Job Requirements:

• You can be a newly qualified Occupational Therapist or come from an experienced background and will need to be registered with the College of Occupational Therapists and HCPC.
• The role requires you to be mobile and as such you will need to hold a driving licence that enables you to drive in the UK.
