Job Description:

This is a full-time and work-at-home Telephonic/Digital RN Case Manager. Employees must conduct business in a secure area in their home, ensuring that discussions are confidential and cannot be overheard by others. Employees’ work space must be private and without any distractions.

Job Responsibilities:

  • Builds solid working relationships with internal team members, internal and external partners, customers and providers
  • Works independently and effectively communicates to internal and external customers in a virtual, telephonic environment
  • Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual’s health needs within their case load
  • Establishes customer-centric SMART goals and interventions to meet the customer’s needs and drive outcomes
  • Promotes quality cost-effective outcomes managing care needs through the continuum of care utilizing effective verbal and written communication skills and consumerism approach through education and health advocacy to our customers
  • Manages/coordinates an active caseload of telephonic/digital case management cases
  • Uses primarily telephonic interactions/digital technology to promote the improvement of health outcomes to customers and assist those experiencing the burdens of illness and injury, through the case management process

Job Requirements:

  • Three years full-time equivalent of direct clinical care and acute care required – not including clinical rotation
  • Access to self-provided high-speed broadband wired internet with a modem/router. Cannot support satellite-based internet or hot spots
  • Excellent communication skills including telephonic (verbal) and digital (messaging, emails)
  • Bilingual in English/Spanish a plus
  • Experience with direct customer communication (written and oral)
  • Case Management Certification preferred
  • Bachelor’s degree preferred
  • Strong computer skills in Microsoft word, Excel, Outlook, and ability to perform thorough internet research as well as toggle between systems, ability to talk and type effectively in real time.
  • Eligibility to obtain additional state licensure based on business need and required by Cigna. Must be able to obtain a California RN licensure for this role
  • Active unrestricted Registered Nurse (RN) license required in State of Residence
  • Excellent time management, organizational, research, analytical, interpersonal skills.
  • Diverse Clinical experience preferred
